Maximizing Independence at Home

The goal of this study is to develop cost-effective ways to deliver dementia care to older adults with memory disorders who live in the community. This project, known as MIND (Maximizing Independence at Home), has developed a person-centered, coordinated care model for older adults with memory problems and is seeking to determine if this approach enables individuals to remain at home longer, with a higher quality of life, than standard methods of care.
